Harborview Medical Center has an outstanding opportunity for Full-Time, MENTAL HEALTH PRACTITIONER for the High Utilizer Program. Under general supervision of the Mental Health Practitioner Lead, this position provides intensive engagement, care-management, linkage and follow-up services for individuals who have been treated in the Emergency Department (ED) and Psychiatric Emergency Services (PES). This program provides intensive case-management services to high utilizers of ED and PES services. This position and program is funded by King County RSN contract. The position will report to the ED/PES Liaison and Case Management Lead.
This position will be responsible for identifying and referring high utilizer clients of the Emergency Department for service coordination with King County Mental Health and Chemical Abuse and Dependency Services. This position will provide assertive outreach and engagement services, to facilitate linkage and coordination with on-going services and community supports. The MHP will be expected to be able to work with a wide variety of patients and be sensitive to the culture, religion, language, age, gender, and sexual orientation of the clients we serve.
CLINICAL AND PROGRAM RESPONSIBILITIES
Provide intensive case management and advocacy services for ED/PES clients to ensure linkage with community resources
Work with patients and their social networks as appropriate
Assess patient needs and determine interventions
May assist clients obtaining housing and funding as necessary to prevent inappropriate use of crisis services
Facilitate diversion from voluntary and involuntary psychiatric hospitalization when clinically appropriate
Work in collaboration with KC staff assigned to the ED/PES to create crisis plans and ensure effective linkages to post-ED services
Identify challenging clients who have behavioral health problems and are high utilizer of the ED
Participate in case reviews as appropriate
Provide linkages to services from mental health, substance abuse, developmental disability and other behavior health services appropriate for patients discharged from the ED.
Maintain active case load, size to be determined by supervisor.
Complete data logs as assigned
Chart patient contact in the medical record
Develop and implement patient care plans,
Collect data for evaluation and quality improvement as assigned
Attend staff meetings as appropriate
ADMINISTRATIVE RESPONSIBILITIES
Provide coverage for other team members as assigned
Represent the needs and concerns of the team at relevant meetings
Meet productivity standards as appropriate
Participate in community program planning
Maintain required daily entries in computer log
Complete payroll documentation/leave requests in timely manner
Attend ED Unit Meetings as required
Participate in HMC Disaster Drills and required HMC training and orientation
Provide consultation to referring practitioners and other members of treatment community as needed to assure continuity of care for program clients
Assure regulations related to client confidentiality are maintained
TEACHING/TRAINING
Provide consultation and training to HMC staff, ED nursing/medical staff and EMS personnel as requested
Act as Practicum instructor as assigned
Act as mentor for newer staff as requested
STAFF DEVELOPMENT
Participate in individual or group supervision as negotiated with supervisor/manager
Participate in HMC ongoing educational opportunities as appropriate
Maintain appropriate State of Washington license or registration.
Obtain training and education relevant to the needs of program and populations served
Obtain continuing education to stay current with trends in mental health, regulations and other pertinent information
MISCELLANEOUS
Testify in judicial proceedings in response to subpoenas
Perform other duties as assigned

REQUIREMENTS: A Master's Degree in behavioral sciences, such as psychology, psychiatric nursing, occupational therapy, vocational or educational counseling, or social work OR Bachelor's Degree in nursing or a behavioral science AND three years' of experience in the direct delivery of professional mental health care. Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration.
